60 year old male with DOV in left eye

This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. 

Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.

HISTORY OF PRESENT ILLNESS :-

Patient was apparently asymptomatic six months back.Later he developed direction of view in left eye.He came to the hospital for the treatment.It was incidious in onset and slowly he developed a blurred vision. Later it is gradually progressive.

HISTORY OF PAST ILLNESS :-

 No Htn/dm/asthma

PERSONAL HISTORY :-

Diet : mixed

Appetite: normal

Bowel & Bladder : regular

Addictions : none

Allergy : not known

FAMILY HISTORY :-

No known family history.

GENERAL EXAMINATION :-

Patient is conscious ,coherent ,cooperative well orientated to time ,space and person.

No pallor ,cyanosis ,lymphadenopathy, icterus ,clubbing ,oedema.

VITALS :

Temp - afebrile

BP - 120/80 mm hg

Pulse rate - 86 bpm

Resp rate - 17 cpm

Sp O2 - 97%

SYSTEMIC EXAMINATION :-

CVS -

S1,S2 heard.

RESPIRATORY SYSTEM -

No dyspnoea

No wheeze

ABDOMEN -

Scaphoid shape

No tenderness

No palpable mass

Bowel sounds present

CNS -

No focal neurological defects

TREATMENT :-

E/d Ciplox
